   Four states mandate IVF coverage and have one clinic or none

   Half of the 422 reporting U.S. fertility clinics sit in six states. At the other end, eleven states have one clinic or none — and four of those legally require insurers to cover IVF. Insurance law turns out to be a poor predictor of whether there is a clinic near you.
